The Fashionable Megalomaniac is a community-created cosmetic item for the Medic. It gives the Medic a pair of team-colored goggles and a short beard, as well as adding a scar which runs across the Medic's right eyebrow and cheek-bone.
The Fashionable Megalomaniac was contributed to the Steam Workshop.

Trivia
- "Megalomaniac" is a term used to describe a person who has delusional fantasies of power, relevance, or unlimited power.
- The design of the item is based off of the stereotypical look of evil scientists.
- The scar added to the Medic's face by this cosmetic is reminiscent of a dueling scar. This form of scar was fashionable with German and Austrian youth both before and during the early part of the 20th century.
