The Clockwerk's Helm is a promotional cosmetic item for the Engineer. It is a half-helmet with glowing, team-colored goggles that fit over the Engineer's default goggles and a metal jaw, similar to the face of Clockwerk, also known as Rattletrap, a hero in Dota 2.
It was rewarded to participants of the “Defence of the Ancients 2” championship in 2011.
In all Likely hood, and in accordance with the Trading Website, Backpack.tf, There are no Clockwerk's Helms in any player's backpack, meaning that either There is a database error on Backpack.tf, or more likely, those Dota 2 Competitors have never launched TF2.
