The Vintage Tyrolean is an unlockable headwear item for the Medic. It appears as a darkly team-colored felt cap with a braided cord band around the crown. To the left side of the hat, a formal-looking crest and a feather are added as trim, both of which are also team-colored. The narrow brim slopes downward towards the front, and is folded flat against the crown in the back.

Trivia
- The Tyrolean is the name for the particular style of hats made in the County of Tyrol in Austrian Alps.
- The International Red Cross and Red Crescent Movement have their main headquarters in Geneva, Switzerland, possibly a connection to why the Medic got this hat.
- Humorously, if you owned a Vintage Tyrolean prior to the Polycount Update, it will be named a "Vintage Vintage Tyrolean". This has been named a "double-vintage" by the community.
- The Tyrolean hat is also know as the Tyrol and the Bavarian cap, it is apart of the signature articles clothing that is worn at Oktoberfest.
- The Tyrolean is one of the few random drop hats that still lacks a description.
- Though the word Vintage is in the name, it is one of two hats whose Vintage versions of themselves still display as "Vintage Vintage Tyrolean", the other being the Vintage Merryweather.
