The Skull Island Topper is an unlockable headwear item wearable by all classes. It is a ram's skull that is missing the left horn and has the right eye intact. The Skull Island Topper is the hat worn by Merasmus.
The hat can be obtained by earning the A Lovely Vacation Spot Necromannchievements by reaching the hat on Skull Island in Ghost Fort.
