The Horseless Headless Horsemann's Head is a headwear item available to all classes. It is awarded to all players who earn the Sleepy Holl0WND achievement, by killing the Horseless Headless Horsemann that spawns in Mann Manor. It is available during the Halloween events only.
While wearing this hat, the player is immune to the Horseless Headless Horsemann's "Boo" taunt.

Trivia
- Despite covering the whole head, items such as the Camera Beard can still be worn with it equipped.
- There is a slight clipping problem when this hat is worn on Pyro. The gas mask's filter will poke out from the mouth of the hat.
- There is a slight clipping problem when this hat is worn on Scout. As he runs, and his dog tags fly behind him, they will poke through the back of the hat.
- Its level isn't randomly generated upon drop. It is always 31, presumably because Halloween is on the 31st of October.
- The eyes and mouth glow in the dark, like the bulb of the Mining Light or Earbuds.
- Unlike other hats, the Horseless Headless Horsemann's Head does not come off when a player dies.
- The Horseless Headless Horsemann's Head is the first hat to completely seal around the classes' heads, meaning all features of the head are completely covered.
- The Horseless Headless Horsemann's Head is first hat to give any kind of protection (except for the polycount hats, when worn with the rest of their set items), which in this case grants immunity to Horseless Headless Horsemann's 'Boo' taunt.
