Item set-urile au fost introduse in actualizarea Mann-Conomy. In functie de set, un efect cosmetic poate sa fie aplicat pe purtator cand toate acestea sunt echipate. Sunt 68 de seturi in total; cei cinci castigatori de la concursul Polycount Pack, trei seturi de la Craciunul Aistralian, opt seturi de la Über Update, setul Dr. Grordbort's Victory Pack, setul Swashbuckler's Swag, noua seturi din Very Scary Halloween Special, trei seturi din Craciunul Australian 2011, seturile secrete Eliminating the Impossible si Dual-Purpose Fruit, trei seturi din Actualizarea Pyromania, setul The Grand Duchess, unsprezece seturi din Scream Fortress 2013, unsprezece seturi din Actualizarea Love & War si zece seturi din Scream Fortress 2014.
Polycount Pack-ul necesita ca jucatorii sa aibe setul de cosmetice echipate ca sa activeze bonusul. Toate celelalte seturi, in timp ce acelea necesita anumite cosmetice, nu detin un set bonus. Toate seturile cosmetice care pot fi fabricate folosesc 4 Refined Metal si o arma din setul dorit (de exemplu, 4 Refined Metal-uri si un Soda Popper vor crea un Bonk Boy).
Majoritatea itemelor sunt proiectate in jurul unie strategii care se bazeaza pe armele care apartin acelui set. De exemplu, setul One Thousand and One Demoknights pentru Demoman este proiectata special pentru o lupta cu arme de tip malee, in timp ce setul Airbone armaments pentru Soldat este proiectat pentru ati arunca inamicii in aer si ai omora inainte sa aterizeze.
Daca o parte dintr-un set este echipata , o lista cu itemele care mai sunt echipate iti capare cand acesta este inspectat sau se poate vedea si din Meniul de Loadout sau de la alti jucatori. Spre deosebire de restul, lista din setul Eliminating The Impossible si Dual-Purpose Fruit raman invisibile pana cand toate itemele sunt echipate. Cand setul este complet, efectele cu care te beneficeaza sunt deasemenea listate.
