There are 16 separate medals, but they share 4 models with differing appearances depending on how the wearer's team ranked in the Season (First, Second, Third or Participant), and which division they played in (Open, Access, Central or Elite). The names differ depending on the rank and division of their team.
These medals were contributed to the Steam Workshop.
