Scourge of the Sky-ul este un obiect cosmetic creat de comunitate pentru Medic. Este o uniformă Germană de pilot din era Primului Război Mondial colorată ca și echipa jucătorului și cu detalii și catarama centurii cu un simbol de Team Fortress 2, toate fiind colorate ca și echipa jucătorului. Îi dă și un guler gros de blană la paltonul standard al lui Medic.

Scourge of the Sky a fost contribut pentru Atelierul Steam.

  • "The Scourge of the Skies" a fost porecla lui Manfred von Richthofen, zis și Baronul Roșu, care s-a luptat cu Fortele Aeriene in timpul Primului Război Mondial.
  • Emblema vospibilă a Medicului de sub guler este cunoscut ca și colier (English), un tip de decoratiune pentru făcută pentru a fi purtată împrejurul gâtului, mai degrabă decât atârnând de pe piept.
