The Cotton Head is a community-created headwear for the Pyro. It is a team-colored trilby hat with a darker, paintable band and two badges. The team-colored one has square shape, a lettering that reads "Rattle Me Tones" and a generic death skull and crossbones with three music notes, while the other one (orange for RED or yellow for BLU) has round shape, a lettering that reads "Mann Club" and one note.
The Cotton Head was contributed to the Steam Workshop.

- "Cottonhead" is an idiom used to describe an old person with white hair, often in a derogatory fashion.
- According to creators, the name is a reference to the track Cotton Heads by French band Caravan Palace.
- The lettering "Rattle Me Tones" may be a reference to a commercial of the toy, "Rattle Me Bones" which coined and popularized the term.
- This item, together with the Soot Suit, was a part of Segab and MultiTrip's "15 Minutes of Flame" Pyro set.
